Specialized fostering program for children with disabilities

ACSC provides medical foster care in licensed homes with capable foster parents who can perform medical care and promote health. ACSC’s program offers comprehensive support including 24/7 medical support, home visits, referrals, reunification assistance, and cultural learning opportunities. The program follows a family-centered approach, which has been linked to positive outcomes, such as improved health care and emotional support for the foster child.

  • Medical and social service support both during the initial placement, and throughout the foster child’s stay with their foster parents
  • Competent healthcare providers for the foster children, including a foster parent with medical knowledge and experience
  • Coordination of care for the children with medical specialists, hospitals, equipment providers, therapists, health facilities, schools, foster care staff, and families
  • A 24-hour on-call medical support line is available no matter the time of day or day of the week
  • Home and community visits for assessment and support
  • Referrals for needed resources
  • Assistance with reunification
  • Culture preservation learning opportunities

ACSC’s medical fostering program prioritizes a family-centered approach to care, rather than institutionalization in a hospital or facility. The licensed medical foster care homes provide family-centered, accessible, continuous, comprehensive, coordinated, compassionate, and culturally effective care. Studies show that specialized medical homes result in positive outcomes for children with special health care needs, including improved family-centered care, reduced health care utilization, increased emotional attachment and support, and improved life skills as they age.

ACSC is committed to establishing strong partnerships with regional and agency stakeholders to ensure the highest quality of care for foster children and to provide support in improving the social services sector’s infrastructure and processes.

ACSC’s consultation services may include (and not limited to);

  • Assisting with the various agencies and healthcare regions to prioritizing children’s care
  • Medical and social services; data research and data analysis, providing possible recommendations/outcomes
  • Client advocacy and/or representation
  • Development of projects or care plans and strategies that are evidence based
  • Educational/training assistance with medical needs or interventions (physical, mental, and /or behavioral)
  • Social Worker support and recommendations with training, education, and various policies and procedures
  • Analysis and reviewing Social Work documentation and care plans
  • Assistance with case management and disposition issues
  • Assistance with creating and implementation of new policies and procedures, new community needs/programs and initiatives
  • Analysis of grievance concern reporting and resolution process
